Please take a moment to download and complete any forms that apply to your child. Once filled out, you can submit them directly to us — it’s the first step to helping your little one Shine Bright! ✨

📄 Important Notice About Required Forms
All required forms must be completed before your child can attend Shine Bright. These forms are required by the state, and all information must be fully completed to ensure compliance with state laws and regulations.
New Enrollment: If you need all forms, please select the “All Forms” option.
Forms can be completed on a computer or laptop, saved, and then emailed to shinebrightcumberland@gmail.comonce finished.
If you prefer, you may also download all forms, print them, complete them by hand, and either hand-deliver or mail them back to Shine Bright.
Please note: Electronic signatures are not enabled. After your forms are reviewed, all required signatures will be completed in person.
Submitting these forms is required for your child to participate in any Shine Bright program.
Important: Forms must be completed on a computer or laptop and cannot be filled out on a phone. While you may download and view the forms on a mobile device, they are not fillable electronically on phones.
Parents are required to complete and submit all necessary forms to Shine Bright prior to their child’s enrollment and participation in any Shine Bright programs or activities.
